I've always wondered that, how it doesn't cut the shoulder. And then I remembered swords aren't that sharp, and as long as it isn't moving fast there isn't a reason for it to be able to cut anything. Also probably they rest it on the flat of the blade. Makes sense with European blades that are symmetrical.
